2. Print, complete on pen and paper or fill the application out digitally with a PDF reader and print. **Most mobile devices will have issues completing the application digitally. We recommend using a PC or Mac to complete the application digitally. ** 
3. Sign your completed Application and scan to a file and use the "save as" function and rename "All Game App".
4. Then complete the Application Submission Form to the right on this page and use the upload button to upload your completed and signed Application Form or send via email or fax:
